Achieving Better Health in DFW
Recently Prism Health North Texas was invited to present at the annual Dallas Fort Worth Case Management Society of America (CMSA) Conference in Arlington TX. The 2018 conference theme was “Case Management: Constructing Better Outcomes” and PHNTX staff presented on “HIV Infection – Achieving Better Outcomes in the Community. The focus of the conference was to empower case managers by sharing tools and information to help them to be the most effective with their clients.
Justin Vander, Community Partners/Digital Media Director, provided an overview to attendees about the broad array of provided by PHNTX.
Alex Ortega, Project Connect Program Manager, presented on the importance of HIV testing across the DFW community and the value of testing for other sexually transmitted infections which have high prevalence rates in North Texas
Benjamin Calloway, Case Management Supervisor II of the Home Again team, provided examples of especially challenging barriers to care faced by people receiving case management. Ben walked the group through specific strategies that have proven consistently effective in providing care and empowering the client to get linked to and stay in care.
Brashad Young, Behavioral Intervention Specialist – PrEP, discussed PrEP and how it provides people with the opportunity to take control of their own health. He elaborated on how PrEP works, which communities would most benefit from knowledge and access to the medication, and how it fits into the care strategy of Prism Health North Texas.
